dictation question - medical transcription

When diagnoses are dictated and numbers are not dictated should these be numbered.

Example:
Dictated. Open wound of the right lower extremity comma complicated comma healed edema dicoid lupus.


should it read

Transcribed. 1. Open wound of the right lower
extremity, complicated, healed.
2. Edema
3. Dicoid lupus

or
Open wound of the right lower extremity,
complicated, healed. Edema. Discoid
Lupus

Formatting of medical transcription

I used to transcribe for some dermatologists a few years back and I used both of your examples according to how it was dictated by the doctor.

Sometimes the doctor would just dictate the medical terms one after the other, so I would just type them out and put commas between them.

Then if the doctor specifically dictated point 1. then medical term or sentence, point 2. then medical term or sentence, etc, then I would format it as per your example two.

Maybe you can ask who you're transcribing for what format they prefer?

I don't know if there is a right or wrong way, could be just a preference for doctors.

Hopefully some seasoned medical transcribers can comment here.
